We the staff at On Pointe! Dance Studio recognize our obligation to make sure our students and their parents are aware of the risks and hazards involved in the sport of dance. By signing this waiver you release On Pointe! Dance Studio and all of its employees from all claims on account of any injury which may be sustained by your child while attending any dance class event associated with On Pointe! Dance Studio or outside performance. In signing this waiver you also acknowledge your responsibility in paying monthly tuition any associated costumes entry fees for performances or competitions and all other communicated costs involved. You also affirm you now have and will continue to carry proper primary medical health hospitalization and accident insurance which you consider adequate for the protection of both your child and On Pointe! Dance Studio.

I understand that tuition is due on the 1st of each month and is considered past due after the 10th. A $10 late fee will be charged if payment is not received by the 10th of the month. A $50 costume deposit will be due before Christmas break and can be paid anytime prior. A $35 recital fee will be due before the fall recital for each family. I understand that any outstanding balance must be paid in full prior to fall recital for my dancer(s) to be eligible to participate in the recital.

On Pointe! Dance Studio uses photography/videography on many occasions throughout the year. By signing this Photo Release you as guardians of your child grant On Pointe! Dance Studio permission to photograph/video your child during dance class and at special dance events to be used at On Pointe's discretion.
